Health Professional Award for Volunteer Service
To honor a volunteer who has made an outstanding contribution to the American Diabetes Association outside of their professional area of practice/interest.
Criteria for Selection
- A volunteer physician, scientist or allied health professional who has demonstrated outstanding service and commitment to the American Diabetes Association above and beyond their area of professional expertise.
- An individual may be nominated or may wish to submit a self nomination.
- Incomplete nomination forms will not be considered.
