To honor an individual, group or organization that has made a demonstrated national impact through the promotion of health awareness, wellness and/or disease prevention.
Criteria for Selection
- An individual, group or organization that has made a significant contribution to increasing public awareness of health related issues at the national level, including overall health awareness, wellness promotion or disease prevention.
- Consideration for this award is not limited to diabetes-specific work; however, nominations with a focus on diabetes will receive priority.
- An individual may be nominated or may wish to submit a self nomination.
- Incomplete nomination forms will not be considered.
