To honor outstanding lifetime achievement in the cause of diabetes.
Criteria for Selection
- Volunteer who is not a physician, scientist or allied health professional whose dedication and efforts have made a significant contribution to the fight against diabetes.
- Contribution could be through leadership, fundraising, advocacy, research funding, and/or as a spokesperson (or any combination of these).
- A minimum of 10 years of service in the cause of diabetes is required for consideration. Longer service in the cause may receive priority.
- An individual may be nominated or may wish to submit a self nomination.
- Incomplete nomination forms will not be considered.
