To honor a volunteer leader who has made an outstanding contribution to the American Diabetes Association.
Criteria for Selection
- Volunteer who is not a physician, scientist or allied health professional whose leadership, efforts and service have made a significant contribution to the American Diabetes Association.
- An individual may be nominated or may wish to submit a self nomination.
- Incomplete nomination forms will not be considered.
