Here are some tips to help you make the most of your visit to your health care provider. Here's what to do.
 
Give information. Talk with your health care provider about your blood sugar levels, what you eat, your activities, how you're feeling, and any problems or concerns.
     
  Ask questions. Your health care provider may not always remember to give you all the information you need. In between visits to your provider, write down your questions. Bring your list with you and ask each question while you're there.
     
  Make a plan for change and talk it over with your health care provider. Is there something about your diabetes care that's hard for you? Or something you'd like to change?
   
What's your goal? For example, is your goal to be more active?
    What do you need to do first?
    What's your plan (what will you do, when, and for how long)?
    What's one thing you can try this week to start working toward your goal?
    How will you reward yourself?
     
  Think about your plan after you've tried it. How did your plan work out? Is there anything you can try to make it work better?
