The goal of the 3rd Annual Disparities Partnership Forum is to highlight promising practices that address type 2 diabetes and obesity in high-risk populations. Also, to introduce key elements of coalition-building for addressing disparities in healthcare at the community level.

November 19th - Closing Speaker: Kyu Rhee,MD, MPP, FAAP, FACP, Chief Public Health Officer, Office of the Administrator, Health Resources and Services Administration -
Learn about what other non-profit organizations, for-profit companies and community- based organizations are doing in high-risk communities to reduce the incidence of obesity and type 2 diabetes

Participate in small-group discussions designed to:
- Explore how to use promising practices (see abstracts) in your community/work setting
- Identify solutions and actions that will eliminate disparities in health care at the community level -
Network with others working on this important issue and find out what you can do to stop diabetes in your community
