What are the Benefits of ADA Education Recognition?

Education Recognition by the American Diabetes Association, the largest and most widely known organization in the field of diabetes:

  • Identifies quality diabetes self-management education services that meet the National Standards for Diabetes Self-Management Education
  • Enhances marketing through use of the ADA Education Recognition logo
  • Meets criteria for Medicare reimbursement
  • Stimulates referrals from local ADA offices
  • Receive free publicity on ADA's Web site
  • Obtain discounts on ADA publications and books

What are the National Standards?

The 6th Edition ADA Education Recognition application is based on the National Standards for Diabetes Self-Management Education published in the May 2000 issue of Diabetes Care. (PDF files require Adobe Acrobat Reader). Although there are some changes in the requirements for ADA Education Recognition, the application process has been streamlined for your convenience.

How Do I Apply for Recognition?

What are the Requirements?

The May 2000 National Standards for Diabetes Self-Management Education are the framework for the 6th Edition ADA Education Recognition application requirements. In summary, the structure, process, and outcomes of the diabetes education services must include:

  1. one sponsoring organization and a qualified coordinator
  2. an advisory system that annually plans and evaluates the services offered, and reviews the participants' outcomes
  3. instructional team that include at least a registered dietitian (RD) and a registered nurse (RN) who have continuing education and experience in both diabetes and behavioral teaching/counseling skills
  4. instructional staff that include at least two health care professionals from at least two different health care disciplines who have continuing education and experience in both diabetes and behavioral teaching/counseling skills
  5. a written curriculum with measurable learning objectives based on the National Standards 10 content areas (See Standard 7)
  6. participant education records that document individualized assessment related to the National Standards content areas, education plan with learning and behavioral objectives, interventions, evaluations, and team collaboration
  7. tracking of participants' behavioral and other outcomes as part of a continuous quality improvement (CQI) process to evaluate the effectiveness of the diabetes education services
  8. Coordinator and staff do not need to be CDE. See the application instructions for the requirements for the coordinator and for the Continuing Education requirements for non-CDE Instructional Team Members and Instructional Staff.

6th Edition Application Process

Log on to this ADA Education Recognition Web site to access the 6th Edition application and instructions at no charge. Watch the Web site for possible changes that may occur at any time. Print out the Application Form Template and manually complete this form prior to entering the data into the online application. Do not begin to enter data into the online application until you read the Instructions, meet the requirements for the full 6 month data period and have completed the Application Form Template.

When the program meets the Standard in all aspects you begin to collect data on the operations of the diabetes education service for a six-month period. Data requirements include demographics, oversight/planning, instructors, curriculum, education records and outcomes. At the end of the six-month data collection period, complete the current Application Form Template, log on to the ADA Education Recognition Web site, complete the online application and submit over the Internet. In addition to the online application, supporting documentation must be mailed to the ERP office. The required documentation is described in the Instructions and in the Support Documentation Package Checklist. The documentation supporting the application must be kept accurate and up to date and available for an audit anytime during the three years of Recognition. All education services receiving ADA Education Recognition are subject to random onsite audits any time during the 3 year Recognition period and/or paper audits at the time of the online application.

6th Edition applications will be scored on a pass/fail basis. With one exception, in order to pass, you must have entered "yes" into all of the "yes/no" decision boxes of the on-line application. You may answer "no" to the curriculum content area, "Preconception care, management during pregnancy and gestational management." Any other "no" answer will fail your application. You must have documentation to support every "yes" answer. (When you review your application note that it is OK for CDEs to have a NO in the Continuing Education boxes and non-CDEs will have a YES in one and a NO in the other.)

Multi-Site Recognition

To be considered one ADA Recognized diabetes education service operating at multiple sites, the following must be in place:

  • One sponsoring organization that supports the diabetes education services within a state. For a primary program site that resides in a metropolitan area that is located on the border of two neighboring states, the program is permitted to have sites in this metropolitan area of the adjoining state.
  • Standards and criteria met at each site
  • One advisory body that meets annually with all sites represented
  • Qualified faculty at each site
  • One coordinator who oversees the planning, implementation, and evaluation of the DSME entity/program with on site supervision at all sites
  • Appropriate curricula and resource materials at each site
  • Annual review and program plan addresses each site
  • If the service is provided at another address this is an additional site and requires a multi-site application with all application criteria met at each site.

The 6th Edition application records the information required from each site. A certificate of ADA Education Recognition is issued for each site Recognized under the sponsoring organization.

Submission Deadlines

All 6th Edition applications must be submitted to the National Office within three (3) months of the end of the data collection period. The last date of the data collection period may not be more than three (3) months prior to the actual date you apply online. The data period must be exactly 6 months. The period between the last day of the data period and the date you enter the application online must be less than 3 months.

Applicants renewing current ADA Education Recognition should plan to submit the online application at least three months before their current Recognition expires.

Fees

The fee for all 6th Edition applications is $1100 for the initial site + $100 for each additional site requested at the time of application. Additional sites may be added at any time during the 3 year Recognition period. See Adding Additional Sites to Current Recognition below. The application fee is not refundable under any circumstances. Payment can be made either by check (the number must be entered in the online application but the actual check should be included in the Support Documentation Package) or credit card (information must be entered into the online application -- credit card payment will not be accepted separately from the online application).

Support Documentation

Complete the current Support Documentation Package Checklist prior to submitting the online application to be certain all required materials have been gathered and that everything is current and complete. The support documentation package should be sent after completing the online application and must be received at the ERP office within 14 days of the submission of an online application.

Adding Additional Sites to Current Recognition

During the three-year Recognition period, a diabetes education service may submit a 6th Edition application for Recognition of an additional site under the current Recognition for a fee of $100.00 as of 7/1/03 per site. The application fee is not refundable under any circumstances. The Recognition period for an added site will be from the date of successful application of that site through the expiration date of the primary program Recognition.

If additional sites are added at the time of extension of Recognition, the program coordinator will be asked to choose which set of dates to use for the program. Recognition may begin either the on date the application is entered online (assuming all is OK this is the first date the new site(s) might be Recognized) or the dates may continue from the expiration date of the previous Recognition.

What if My Application is Not Complete?

If upon review of your application and documentation, your application is deficient in any way, you shall be notified by e-mail or fax describing the deficient items. You shall be allowed no more than 30 days from the date of notification to submit the items to complete your application. If you do not or are unable to complete your application by 30 calendar days after notification, your application shall be failed and you shall lose your application fee. It is your responsibility to contact the ADA ERP at 888/232-0822 to schedule the use of the "failed reapply" feature of the application.

What if My Application is Approved?

The program coordinator will receive an email notification of approval. Recognition is awarded for a three year period and the email will include the dates of the current recognition.

The actual certificate, along with the ADA Education Recognition logo and the rules for use of that logo, will be sent and should be received within 30 days of the official notification of approval.

How Will My Certificate Read?

The name of the site or program, if it is different from the sponsoring organization, will appear as line one on the certificate of Recognition. The second line will be the name of the sponsoring organization. The third line will be the city and state of the site. If the name of the site/program is the same as the sponsoring organization, a 2 line certificate will be issued. All multi-site certificates will have 3 lines with the name of the site/program, the name of the sponsoring organization, and the city/state of the site/program. All information on the certificate will appear exactly as it appears in the application. Be certain spelling and punctuation is accurate.

If your program has a "name" such as "Living with Diabetes" and you want that to appear on the certificate it must be entered as the site name on the application.

How Can I Use the ADA Education Recognition Logo?

When Recognition is awarded, camera-ready copy of the approved ADA Education Recognition logo is sent to the diabetes education service for use in publications or advertisements with the following instructions:

  • All proposed uses of the ADA Education Recognition logo must be e-mailed to erp@diabetes.org or faxed to (703) 549-1715 by the coordinator to the National Office for pre-approval by the Education Recognition staff.
  • Staff will provide written or verbal approval to the coordinator and a copy will be kept in the ADA National Office files. The approval is valid for multiple uses of the same format such as revised schedules or flyers. If an approved use is changed substantially, separate approval must be sought. It may not be used on letterhead, business cards or forms.

The ADA Education Recognition logo must be displayed when saying that diabetes education services are ADA Recognized. Only those services that are ADA Recognized can be advertised (i.e. if a healthcare provider has multiple sites, only those that are Recognized may be listed in any information that uses the ADA Education Recognition logo). It must also be clearly indicated that only the education or self-management training (not treatment) services are Recognized.

  • Requests for electronic versions of the ADA Education Recognition logo and the rules for use of the logo should be sent by the coordinator to erp@diabetes.org.

  • The ADA Education Recognition logo is the property of the American Diabetes Association and use of the name, logo, and marks is not permitted without the written or verbal consent of the American Diabetes Association.

  • Diabetes education services that are no longer Recognized must not use the ADA Education Recognition logo or previously approved materials displaying the ADA Education Recognition logo.

What Is Required After Recognition Is Achieved?

Programs must continue to meet the National Standard during the 3 year Recognition period. See below under Annual Status Report for a list of requirements.

ADA must be notified within 30 days if there is a change of coordinator or if there is any change in the Sponsoring Organization, address or contact information posted on the ADA web site. There must be a coordinator at all times. An interim coordinator may be assigned coordinator duties if an individual leaves the position prior to a replacement coordinator assuming the duties.

On-Site Audit - A program might be randomly selected for an on-site audit at any time during the 3 year Recognition period. If a program is selected the coordinator will be contacted by phone approximately 2 weeks prior to the audit date. A package of instructions explaining the process and the materials or information which must be provided will be sent immediately after the phone contact. Refusing an onsite audit will result in immediate loss of Recognition.

Annual Status Report - A report will be mailed to the coordinator during the 1st and 2nd anniversary month of the Recognition. A sample of the form is included in materials sent to the coordinator when the program is Recognized. Completion of this report is required for continued Recognition. The report verifies the following:

  • The program continues under the same sponsoring organization
  • The advisory committee continues to be in place as required
  • The multidisciplinary staff continues to be in place as required
  • Basic and continuing education requirements for all non-CDE instructors continue to be met – Continuing education for all non-CDE instructional staff and team – 15 continuing education hours earned during the past year in any combination of diabetes specific, diabetes related, psychosocial, or educational topics – see application instructions for requirement) (NOTE: Continuing education requirements may be different for the year prior to reapplication for extension of Recognition.  Check the current application to determine the number of hours required in the year prior to reapplication)
  • The program offers all 10 content areas of the National Standards
  • The Program tracks behavioral goals and one other outcome measure for all participants
  • Confirms the name of the coordinator and that all contact information is correct.

Interim Status Report - An Interim Status Report may be required if any information about the program is received by ERP staff which may indicate that the program no longer meets the Standard - this report is sent only if there is a question or concern.
