Your Child's Diabetes Medical Management Plan

The Diabetes Medical Management Plan (DMMP) describes how your child's diabetes will be managed in school.

The DMMP should include:

  • Emergency contact information
  • Your child's need for assistance and/or ability to perform self-care
  • Symptoms and treatment of hypoglycemia and hyperglycemia
  • Specific medical orders for blood glucose monitoring, ketone checks, insulin, glucagon, and other medications
  • Diabetes supplies and storage
  • Meal and snack plan
  • Exercise

Your child's DMMP should be reviewed and updated each school year or any time there is a change in your child's health or diabetes care regimen. You can view a sample DMMP here.
