Pathways for People with Diabetes
The Association EXPO has created a guide to help you get the most out of your EXPO experience. The columns shown in the survey are designed to give you suggestions for how to spend your time at EXPO as well as how you can create a healthy lifestyle at home. Whether you have type 1 diabetes, type 2 diabetes, prediabetes, want to prevent diabetes or are a family or friend of a person with diabetes. All are welcome to participate!
Participation in the Pathways requires you to fill out a survey and turn in to our Pathways table in the front of the EXPO. Be sure to complete either your email (preferred) or phone number so that the EXPO staff may follow up with you in a few months to check your progress on your goals and healthy lifestyle changes.
Surveys will be available at Registration and Pre Registration the day of the event, or you can print yours' out here.
After you turn in your survey you will be able to draw a prize which could include one of the following:
