"I Decide" to Stop Diabetes

Southeastern Pennsylvania, South Jersey & Delaware

   

On Sunday, November 10, 2013 help kick start American Diabetes Month with the American Diabetes Association by participating in our national "I Decide to Stop Diabetes Sunday."

Churches are asked to become a champion for the cause by taking a brief moment during the church service to discuss the seriousness of diabetes, how it is affecting our community and what each of us can do to reduce our risk of diabetes or take better care of ourselves.

Churches can commit to being a part of the "ID Day" event by completing the participation form below.

Please make sure to include a valid mailing address. Materials will be sent via UPS to your location at no cost to you (NO POB please). You may also pick up materials at our office, 150 Monument Road, Suite 100, Bala Cynwyd, PA.

Sign up before October 25, 2013 and your church will be eligible to win various prizes.
