Say Thanks to Your Child's School Staff - Nominations Now Open
The American Diabetes Association's annual Partners in Care Program offers a great opportunity to thank and honor school staff who have provided exceptional support on behalf of your child/teen at school.
To nominate a teacher, nurse, coach, secretary or other school staff who have helped ensure your child/teen's diabetes management needs are met, simply complete our nomination form.
Each nominee receives a personalized Certificate of Appreciation along with a Partners in Care Lapel Pin. Both the certificate and lapel pin can be presented by you and your child personally or be mailed directly to nominees before the end of the school year. There is no cost to participate and you can nominate up to five staff members.
Just fill out this Partners in Care nomination form and send it in.
