Building Successful Diabetes and Obesity Programs
National Conference on Type 2 Diabetes and Obesity in High-Risk Populations
The goal of the 3rd Disparities Partnership Forum was to highlight promising practices that address type 2 diabetes and obesity in high-risk populations. Also, to introduce key elements of coalition-building for addressing disparities in healthcare at the community level.
The meeting was held November 18 - 19 at the Hyatt Regency in Arlington, VA. The keynote speakers were Dr. Garth Graham Garth Graham, MD, MPH, Deputy Assistant Secretary, Office of Minority Health, and Dr. Kyu Rhee Kyu Rhee, MD, MPP, FAAP, FACP, Chief Public Health Officer, Office of the Administrator, Health Resources and Services Administration.
A key feature of the 2009 forum was the presentation of promising practices that can be applied to community efforts aimed at reducing the risk of type 2 diabetes and obesity in high-risk populations, including youth and adults. Thirty-five promising practices received, thirty-four were selected for inclusion in a compilation. Of the thirty-four selected, six practices were chosen to provide presentations during the Forum:
- The Healthy Schools Partnership - American Dietetic Association Foundation
- Kidney Early Evaluation Program (KEEP) - National Kidney Foundation
- Medicare Diabetes Screening Project - Medicare Diabetes Screening Project
- REACH Charleston and Georgetown Coalition: Decreasing Amputation in African American Males - Medical University of South Carolina
- On the Road to Living Well with Diabetes - Joslin Diabetes Center
- The Patient Safety & Clinical Pharmacy Services Collaborative (PSPC) with a focus on the Diabetes and Obesity Teams - HRSA
