PHD Information List
Here is a list of medical information you'll be asked for when creating your Diabetes PHD profile. While not all profile questions will be required, you can improve the accuracy of your results by providing as much information as possible.
- Basic personal information (age, weight, gender, etc.)
- Basic family health history (if anyone had diabetes or heart disease)
- Blood pressure
- Cholesterol levels
- Fasting glucose level
- A1C*
- Health history (whether you smoke, how active you are, whether you've had certain health problems like a heart attack or stroke)
- Current medications related to diabetes, blood pressure, or cholesterol
