Benefits of Insulin Therapy in Type 2 Diabetes

What is the problem and what is known about it so far?

Obese patients are at great risk for problems that can lead to heart disease and diabetes. A lot of evidence suggests that good blood glucose control and earlier use of insulin in obese people with diabetes can lower their risk of health problems.

Why did researchers do this particular study?

The researcher wanted to review diabetes management in obese patients; they wanted to see how physical activity and tight blood glucose control affected these patients.

How was the study done?

The researcher searched articles published between 1985 and 2006 that looked at diabetes management in obese patients.

What did the researchers find?

There is new evidence that tight blood glucose control and earlier use of insulin can help obese patients with type 2 diabetes and reduce their risk for complications. The anti-inflammatory effects of insulin may also protect patients against blood vessel disease. Despite the benefits of insulin therapy, many patients and health care providers are reluctant to start insulin therapy because of concerns about weight gain. However, newer insulin analog products can improve glucose control without causing significant weight gain.

What are the implications of the study?

A good treatment regimen that focuses on diet, exercise, and aggressive use of medicines, including intensive insulin regimens, is the most promising course for reducing long-term complications of type 2 diabetes in obese patients.
